2.Comparative study of the upper and lower limb skin blood flow control mechanisms in patients with essential hypertension

Amaç: Esansiyel hipertansiyonlu (n=33) hastalarda (EHT) deri kan akımı kontrol mekanizmalarının ekstremitelere özgü farklılıklarını araştırmak. Yöntemler: Bu gözlemsel kesitsel çalışmada, yatar pozisyondaki bazal deri kan akımı ve lokal ısısal uyarana (42°C) yanıtlar ön kolun volar ve bacağın gaiter bölgelerinden bir laser Doppler (LD) akış ölçer ile kayıtlandı. Deri kan akımı kontrol mekanizmalarının korelasyon özelliklerini belirlemek için LD sinyalinin fraktal analizi detrended fluctuation analysis (DFA) yöntemi kullanılarak yapıldı. Aynı bir bölgenin ısısal uyarana yanıtları eşleştirilmiş t-testi, farklı anatomik bölgelerin ölçek üsselleri ise tekrarlanan ölçümlerin variyans analizi (ANOVA) ile karşılaştırıldı. Bulgular: LD sinyallerinin fraktal özelliklerini eğimleri (ölçek üsselleri) ile tanımlayan üç doğrusal bölge bulundu. Kardiyak (α) ve kardiyo-respiratuvar (αCR) ölçek üssellerinin lokal ısısal uyaranla (T) değişimleri bacakta (αC=1,49±0,26; αCT=1,62±0,20 p<0,01 ve αCR=0,84±0,29 αCRT=0,42±0,28 p<0,001) koldakinden (αC=1,28±0,13; αCT=1,33±0,13 p>0,05 ve αCR=0,73±0,15; αCRT=0,65±0,018 p<0,05) daha büyüktü. Her iki ekstremitedeki lokal ölçek üsselleri (αL≈αLT~1) arasında anlamlı farklılık yoktu (p>0,05) ve lokal doğrular ısısal uyaranla kendilerine paralel olarak kayma göstermediler. Sonuç: Bu çalışmanın sonuçları, EHT’li hastaların deri kan dolaşımını kontrol eden mekanizmalarının her iki ekstremitede de bozulduğunu gösterir. Bununla birlikte, lokal ısıssal uyarana karşı myojenik yanıt her iki ekstremitede de aynı değildir ve gaiter bölgenin lokal ısısal uyarana yanıtı, volar bölgeninkikinden daha güçlüdür. Bu, ekstremitelere özgü mikrovasküler fonksiyon farklılıklarının posture dayalı yapısal ve fonksiyonel uyumun sonucunda mı gerçekleştiğini belirlemek daha ileri çalışmaları gerektirir.
Objective: To investigate limb specific differences in cutaneous vascular function in patients (n=33) with essential hypertension (EHT). Methods: In this observational cross-sectional study, baseline skin blood flow and the response to local heating were measured with a laser Doppler flowmeter (LDF) from the volar region of the forearm and the gaiter area of the foot at supine rest. The fractal analysis, detrended fluctuation analysis (DFA), was used to calculate the correlation properties of skin blood flow, LDF signal. The paired t-test and repeated measures ANOVA were used to determine the response to local heating and compare the scaling exponents of different anatomical locations respectively. Results: We found three linear scaling regions that describe the fractal behavior of LDF signal with their slopes, scaling exponents. For cardiac (αC) and cardio-respiratory (αCR) scaling exponents, thermal hyperemia (T) induced greater change in the leg (αC=1.49±0.26; αCT=1.62±0.20 p<0.01 and αCR=0.84±0.29 αCRT=0.42±0.28 p<0.001) than in forearm (αC=1.28±0.13; αCT=1.33±0.13 p>0.05 and αCR=0.73±0.15; αCRT=0.65±0.018 p<0.05). Local scaling exponents (αL≈αLT~1) were not significantly different (p>0.05) and, local lines did not shift in parallel with local heating in both extremities. Conclusion: The results of the present study suggest that skin microvascular function is impaired in both extremities in EHT patients. However, myogenic response is not uniform in both extremities and pronounced response to local thermal hyperemia has been observed in the gaiter area compared with the volar region. Further studies are needed to determine if these limb specific microvascular differences is the result of posture-induced structural and functional adaptation.

3.Rapid ultrasound score as an indicator of atherosclerosis’ clinical manifestations in a population of hypertensives:

Amaç: Brakiyal arterin akıma bağlı dilatasyonu (ABD), renal rezistif indeks (RRI), santral arterin retina rezistif indeksi (SARRİ) ve karotis intima-medya kalınlığı (IMK) pre-klinik aterosklerozun iyi bir temsili belirleyicisi olarak kardiyovasküler riskin ultrasonla değerlendirilmesinde kullanıldı. Bu dört parametrenin karşılıklı ilişkilerini araştırdık ve integre skorlamanın hipertansiflerde aterosklerotik hastalığın iyi bir indikatörü olup olmadığını inceledik. Yöntemler: Yüz elli iki ardışık kişi Nisan 2004 ile Nisan 2006 arasında bu çalışmaya alındı. Her hastaya bilgisayarlı beyin tomografisi, koroner arter anjiyografisi, karotis, renal, santral retinal ve femoral arter Doppler ultrasonografi değerlendirilmesi yapıldı. İstatistiksel analizde ANOVA, Fisher test, Pearson korelasyon ve adımsal regresyon analizleri kullanıldı. Bulgular: ABD, RRISA, IMK ve RR birbirleri ile önemli paralellik gösterdi. Multipl regresyon analizinde yaş, nabız basıncı, hipertansiyon süresi dört parametre ile bağımsız olarak ilişkiliydi. Total aterosklerotik hastalığın (AH) 81’i (15 serebro vasküler hastalık, 20 koroner kalp hastalığı ya da miyokard infarktüsü, 22 karotis plağı ve 24 alt ekstremite plağı) ayrıntısı ile kaydedildi. İntegre skorlama kullanarak toplumu üç skorlama bandına bölebildik. En düşük bandda total AH’nın %16’sı ile 87 hasta sınıflandırıldı; intermediyette total AH’nın %30’u 40 hasta; total AH’nin %54’ü 25 hasta en yüksek bandda idi. Gruplar arasındaki farklılıklar önemli idi (p<0,05). Sonuç: Bu integre, düşük fiyatlı ve kolay saptanabilen parametrelerin potansiyel yararı aterosklerotik riskli hastaların sınıflandırılabilmesidir. Bu metod komplikasyonlardan önce tedavinin başlatılması ile serebro-kardiyo-vasküler olay riskinin azalması için pre-klinik basamakta aterosklerozun keşfedilmesinde yararlı olabilir.
Objective: Flow-mediated dilatation (FMD) of brachial artery, renal resistive index (RRI), retina resistive index of central artery (RRICA) and carotid intima-media thickness (IMT) have been used for ultrasound assessment of cardiovascular risk as good surrogate markers of pre-clinical atherosclerosis. We investigated the interrelationship of these four parameters and examined whether an integrated score is a good indicator of atherosclerotic disease in hypertensives. Methods: One-hundred fifty-two consecutive subjects were enrolled in this study between April 2004 and April 2005. Each patient underwent cerebral computed tomography, coronarography, carotid, renal, central retinal and femoral arteries Doppler ultrasonographic evaluation. Statistical analysis was performed using ANOVA, Fisher test, Pearson correlation and stepwise regression analyses. Results: FMD, RRICA, IMT and RRI were significantly correlated with each other. In multiple regression analysis age, pulse pressure, hypertension duration were independently related with the four parameters. Eighty-one findings of total atherosclerotic disease (ADAD were recorded overall (15 cerebrovascular disease, 20 coronary heart disease or myocardial infarction, 22 carotid plaques and 24 low limb plaques). Using an integrated score we were able to divide the population into three scoring bands. In the lowest band we classified 87 patients with 16% of total AD; in the intermediate 40 patients with 30% of total AD, in the highest 25 patients with 54% of total AD. Differences between groups were significant (p<0.05). Conclusion: A potential benefit of these integrated, low-cost and easy-to-detect parameters, is the stratification of patients with atherosclerotic risk. This method may prove useful in discovering those with atherosclerosis in a pre-clinical stage for whom therapy initiated before complications could reduce the risk for a cerebro-cardio-vascular event.

5.Gender specific association of ABCA1 gene R219K variant in coronary disease risk through interactions with serum triglyceride elevation in Turkish adults

Amaç: ATP binding cassette transporter A1 (ABCA1) ters kolesterol transferini kontrol eder. ABCA1 genindeki bazı varyantlar serum yüksek dansiteli lipoprotein kolesterol (HDL-K) ve diğer lipit konsantrasyonları ile ilişkilidir. Bu çalışmada, lipit profili ve koroner kalp hastalığı (KKH) riski ile ABCA1 geni arasındaki ilişkiyi araştırmayı amaçladık. Yöntemler: Seçilen 627 birey (282 erkek ve 345 kadın) PCR-RFLP (polymerase chain reaction restriction fragment length polymorphism) yöntemi kullanılarak ABCA1 R219K polimorfizmi için genotiplenmiştir. İstatistiksel analizler için Ki-kare, doğrusal ve lojistik regresyon, tek yönlü ANOVA ve student t-testi kullanıldı. Bulgular: Çalışmamızda, R219K polimorfizminin plazma lipitleri ve koroner kalp hastalığı üzerine olan cinsiyete özgü etkisi gösterilmiştir. Erkeklerde, KK homozigotluğu artmış plazma LDL-C (p<0,05) ve total kolesterol (p<0,05) konsantrasyonları ile ilişkili iken, kardiyovasküler risk faktörlerine göre ayarlandıktan sonra bu allel taşıyıcılığının yüksek HDL-C konsantrasyonları (p<0,05) ile de ilişkisi olduğu bulundu. Fakat erkeklerde KKH ile R219K polimorfizmi ilişkili değildi. Kadınlarda ise, HDL-K düzeyi ile ilgili olmadan, K alleli %10 daha yüksek plazma trigliserid konsantrasyonları (p<0,05) ile ilişkiliydi. Lojistik regresyon modelinde çok değişkenliler için ayarlama yapıldıktan sonra, R219K polimorfizminin RK genotipi kadınlarda bağımsız olarak KKH için iki kat olasılık oranı göstermiştir (3,80 1,00 %95 CI). Kadın 219RK genotipi taşıyıcılarında, KKH ile TG yüksekliği (>140 mg/dL) etkileşimi gösterilmiştir. Sonuç: ABCA1 geninin R219 alleli, azalmış HDL-K aracılığıyla değil, ancak 219K alleli tarafından yükselmiş TG düzeyi ile etkileşerek heterozigot kadınlarda bağımsız olarak CHD riskine katılırken, erkeklerde böyle bir ilişki göstermemektedir.
Objective: ATP binding cassette transporter A1 (ABCA1) controls the reverse cholesterol transport. Some ABCA1 variants are correlated with serum high-density lipoprotein cholesterol (HDL-C) and other lipid concentrations. We aimed to explore the relationship of ABCA1 gene with both the lipid profile and coronary heart disease (CHD) risk. Methods: Selected 627 individuals of the Turkish Adult Risk Factor Study were genotyped for ABCA1 R219K polymorphism using PCR-RFLP (polymerase chain reaction-restriction fragment length polymorphism) method. Student's t-test, one-way ANOVA, Chi-square test, linear and logistic regression was used for statistical analysis. Results: We demonstrated a gender-specific effect of the R219K polymorphism on plasma lipids and CHD. In men, while homozygosity of the K allele was associated with increased plasma low-density lipoprotein cholesterol (LDL-C) (p<0.05) and total cholesterol concentrations (p<0.05), carriage of this allele was associated with higher HDL-C concentrations (p<0.05) after adjustment for associated risk factors, but not with CHD. In women, however, without being related to HDL-C levels, each 219K allele was associated with 10% higher triglycerides (TG) concentrations (p<0.05). R219K heterozygosity in women independently doubled (95% CI 1.00; 3.80) the odds ratio for CHD risk in regression models, after adjustment for several variables. Interaction of TG elevation (>140 mg/dL) with CHD was demonstrated in female 219RK genotype carriers. Conclusion: R219 allele of the ABCA1 gene independently confers CHD risk in heterozygote Turkish women, not via reduced HDL-C, but interacting with elevated TG expressed by the 219K allele, but not in men.

6.Apparently “low” serum asymmetric dimethylarginine is associated with fasting glucose and tends toward association with type-2 diabetes

Amaç: Serum asimetrik dimetilarginin (ADMA)’nın metabolik sendrom (MetS), tip-2 diyabet ve koroner kalp hastalığı (KKH) ile ilişkilerini bir genel popülasyon örnekleminde inceledik. Yöntemler: Kesitsel ve (2000 kişi-yıllık izlemede) öne dönük analiz. Ölçülmüş serum ADMA düzeyi bulunan 848 yetişkin, üçte bir dilim ya da dikotomize değerler kullanılarak incelendi. ADMA konsantrasyonları valide edilmiş bir ELISA kitiyle ölçüldü. Bulgular: Dikotomize olup düşük (≤0,68 µmol/L) ADMA değerli katılımcılar anlamlı biçimde yüksek açlık glikozu, total kolesterol, apolipoprotein B ve düşük diyastolik kan basıncına sahipti. Yaş, sigara, trigliserit, HDL-kolesterol, C-reaktif protein ve bel çevresini de içeren lineer regresyon analizlerinde ADMA, kreatinin ile anlamlı ve bağımsız ilişki, kadınlarda da glikoz ile ters ilişki gösterdi. Yaş, sigara içiciliği ve bel çevresi için ayarlanan lojistik regresyonda, prevalan MetS, ADMA üçte bir dilimleriyle yalnız erkeklerde bağımsız ilişkili bulundu. Prevalan ve insidan diyabet, orta ve üst üçte bir dilimlerine kıyasla alt dilimde iki cinsiyet birlikteliğinde zayıf ilişki eğilimi sergiledi; prevalan ile insidan KKH ADMA dilimleriyle ilişkili bulunmadı. Sonuç: Kanda görünürde “düşük” ADMA düzeyi açlık glikozu ile bağımsız biçimde ilişkili olup tip-2 diyabet ile de benzer ilişki eğilimindedir. ADMA’nın kalp ve metabolizma hastalıklarıyla beklenen pozitif ilişkilerinin yokluğu, muhtemelen oksidatif stres altında gelişen otoimmün yanıtlara bağlı olup immün yolla ölçümlerden kısmen uzaklaşmasına yol açmaktadır.
Objective: We investigated the association of serum asymmetric dimethylarginine (ADMA) with metabolic syndrome (MetS), type-2 diabetes and coronary heart disease (CHD) in the general population. Methods: Cross-sectional and, at 2000 person-years’ follow-up, prospective analysis. Adults with measured serum ADMA level (n=848) were analyzed using tertiles or dichotomized values. ADMA concentrations were measured by a validated commercial ELISA kit. Results: Dichotomized subjects of combined sexes with low (≤0.68 µmol/L) ADMA values had significantly higher fasting glucose, total cholesterol, apolipoprotein B and lower diastolic blood pressure. In linear regression analyses comprising age, smoking, triglyceride, HDL-cholesterol, C-reactive protein and waist circumference as well, creatinine was significantly and independently associated with ADMA, further in women glucose (inversely). In logistic regression analyses uniformly adjusted for age, smoking status and waist girth, prevalent MetS tended to positive independent association with ADMA tertiles only in men. Combined prevalent and incident diabetes weakly tended to be associated with the lowest (vs mid- and highest) ADMA tertiles in combined gender; and prevalent and incident CHD was not associated with ADMA tertiles in either sex. Conclusion: Apparently “low” circulating ADMA is independently associated with fasting glucose and tends to be so with type-2 diabetes. The lack of anticipated positive associations of ADMA with cardiometabolic disorders is likely due to autoimmune responses operating against serum ADMA under oxidative stress, rendering partial failure in immunoassay.

7.Long-term prognosis of mild functional tricuspid regurgitation after mitral valve replacement

Objective: Functional tricuspid regurgitation (FTR) is the most common type of tricuspid insufficiency and occurs approximately in 30% of patients with mitral valve disease. The major etiologic factor in the triggering of right ventricular dilation and thus causing functional tricuspid regurgitation, is pulmonary artery hypertension secondary to mitral valve disease. We aimed to analyze long-term outcomes of patients with mild tricuspid regurgitation at the time of mitral valve replacement. Methods: Sixty-six patients with mild tricuspid insufficiency who underwent mitral valve replacement were included in this observational retrospective study. Mean follow-up time was 8.3±0.7 years. Patients whose tricuspid regurgitation remained unchanged or decreased following operation were enrolled to group 1 (n=32), patients whose tricuspid regurgitation increased were included to group 2 (n=34) and data were compared statistically with t-test, Mann-Whitney U, Chi-square and Fisher Exact test. Multiple regression analysis was performed to determine independent risk factors for FTR progression. Results: Preoperatively female gender (p=0.02), body surface area (p=0.04), left atrium diameter (p=0.01), functional capacity (p=0.03), right ventricle diameter (p=0.04), and left ventricle mass index (p=0.04) were found to be statistically significant between groups. In the follow-up; functional capacity, grade of tricuspid insufficiency, pulmonary artery pressure, vena contracta width (p<0.001), TAPSE (tricuspid annular plane systolic excursion index) (p=0.04), annulus diameter (p=0.02), right ventricle diameter (p=0.01), left ventricle mass index (p=0.05), and ejection fraction (p=0.02) were found to be statistically different between groups. In multiple logistic regression analysis; preoperative LA diameter (OR=5.05; 95% CI: 1.49-17.12; p=0.009) and female gender (OR=10.93; 95% CI: 1.77-67.31; p=0.01) were found as independent risk factors for FTR progression. Conclusion: This study revealed that mild FTR might advance to moderate to severe grade in more than half of the patients in the follow-up. Thus, surgical approach to even mild FTR should be individualized based on patient’s risk assessment.

8.Angiographically evident atherosclerotic stenosis associated with myocardial bridging (MB) and risk factors for the artery stenosis located proximally to MB: an observational study

Amaç: Bu çalışmanın amacı miyokardiyal köprüleme (MK) ile ilgili koroner arter darlığının ilgili risk faktörlerini bulmak ve MK ile ilişkili aterosklerotik darlığın koroner anjiyografide yaygınlığının kanıtını tayin etmekti. Yöntemler: Başlangıçta, anjiyografi ile tanınmış MK-mural koroner arterli (MK-MKA) 603 hasta Mayıs 2004 ile Mayıs 2009 arasında bu gözlemsel çalışmaya alındı. Tek yönlü ANOVA, t-testi, Pearson korelasyon testi ve çoklu aşamalı regresyon analizleri ilgili risk faktörlerini ortaya çıkarmak için uygulandı. Bulgular: Toplam 644 MK-MKA incelendi. MK’nin distalinde lokalize olmuş lezyonların yaygınlığı MK’nin proksimalindekilerden anlamlı olarak daha azdı [36 (%5,9) karşılık 382 (%62,4), p<0,001]. MK segmentlerinde diyastolik damar çapları referans segmentlerden anlamlı olarak daha küçüktü (p≤0,001). Ülser benzeri lezyon bir MK-MKA’lı hastada bulundu. Multivaryant analiz vasküler bifurkasyon lezyonlarının, non-MK-MKA’nın hastalanmış koroner damarlarının sayısı ve dar olma derecesi, yaş, düşük dansiteli lipoprotein kolesterol (DDL-K)/yüksek dansiteli lipoprotein kolesterol (YDL-K), erkek, diyabetin süresi ve MK-MKA’nın sistolik daralma oranı (SDO) MK’ye proksimal yerleşimli birinci koroner arter darlığının daralma derecesi ile pozitif ilişkide olduğunu telkin etti (p<0,05). Vasküler bifurkasyon lezyonları, non-MK-MKA’nin hastalıklı koroner damarlarının sayısı ve daralma derecesi, yaş, DDL-K/YDL-K, erkek, diyabet ve dislipidemi MK’ya proksimal lokalizasyonlu çok şiddetli koroner arter darlığının daralma derecesi ile pozitif ilişkili idi (p<0,05). Sonuç: Köprülenmiş arterlerin intramural ve distal bölümleri arterin ateroskleroz oluşumu için yasak bölgeler değildir. MK-MKA’nın SDO MK’ye proksimal yerleşimli koroner arter darlığına önemli karar faktörlerinden biri olabilir.
Objective: The purpose of this study was to determine the prevalence of coronary angiographically evident atherosclerotic stenosis associated with myocardial bridging (MB) and to explore related risk factors of coronary artery stenosis located proximally to MB. Methods: Overall, 603 patients with MB-mural coronary arteries (MB-MCAs) diagnosed by angiography initially were enrolled in this observational study during May 2004 to May 2009. One-way ANOVA, t-test, Pearson correlation test and stepwise multiple regression analysis were performed to explore related risk factors. Results: Totally 644 MB-MCAs were examined. Prevalence of lesions located distally to MBs was significantly lower than those proximally to MBs [36 (5.9%) vs. 382 (62.4%), p<0.001]. Diastolic vessel diameters in MB segments were significantly smaller than reference segments(p<0.001. Ulcer-like lesion was found in MB-MCA in 1 patient. Multivariate analysis suggested that vascular bifurcation lesions, the degree of narrowing and the number of diseased coronary vessels of non-MB-MCA arteries, age, low-density lipoprotein cholesterol (LDL-C)/high density lipoprotein cholesterol (HDL-C), male, course of diabetes, and systolic narrow rate(SNR) of MB-MCAs were positively related with the narrow degree of the first coronary artery stenosis (FCAS) located proximally to MBs (all p<0.05). Vascular bifurcation lesions, the degree of narrowing and the number of diseased coronary vessels of non-MB-MCA arteries, age, LDL-C/HDL-C, male, diabetes and dyslipidemia were positively related with the narrow degree of the most severe coronary artery stenosis(MSCAS) located proximally to MB (all p<0.05). Conclusion: The intramural and distal portions of a bridged artery are not the forbidden zone of artery atherosclerosis formation. SNR of MB-MCA may be one of the important decision factors to coronary artery stenosis located proximally to MB.

Objective: The aim of this study was to evaluate the relation between blood gamma-glutamyltransferase (GGT) levels and coronary collateral circulation in patients with chronic total occlusion (CTO). Methods: Two hundred twenty-two patients with chronic stable coronary artery disease (CAD) and CTO were included in this cross-sectional, observational study. Coronary collaterals were graded from 0 to 3 according to the Rentrop method. Patients with grade 0-1 collateral development were regarded as poor collateral group (n=66) while patients with grade 2-3 collateral development were regarded as good collateral group (n=156). Statistical analysis was performed using independent samples t, Mann-Whitney U and Chi-square tests, logistic regression and receiver operator curve analysis. Results: The poor coronary collateral group had significantly higher levels of serum GGT compared to the good collateral group (p<0.001). Multiple logistic regression analysis showed that GGT levels were independent predictors of poor collateral circulation (OR-0.946, 95% CI=0.918-0.9719, p<0.001). The result of ROC curve analysis for GGT was as following: area under the ROC curve (AUC)=0.732, 95% CI: 0.622-0.841, p<0.001. Conclusion: Higher GGT levels are associated with poor coronary collateral circulation in patients with CTO. GGT may be used to predict the grade of coronary collateral circulation in CTO patients with chronic stable CAD.

Amaç: Bu çalışmada stabil koroner arter hastalarında ABO kan grupları ve SYNTAX skoru (SS) ile değerlendirilen koroner lezyon kompleksitesi arasındaki ilişkiyi araştırmak istedik. Yöntemler: Kesitsel çalışmamız 559 stabil koroner arter hastalarından oluşmakta idi. Tüm hastaların ABO kan grupları belirlendi, ek olarak SS, (0-22) düşük SYNTAX skoru, (23-32) orta SYNTAX skoru, (>32) yüksek SYNTAX skoru olarak belirlendi. Sayısal değişkenlerin karşılaştırılmasında Student’s t test, İstatistiksel analizde Student t veya Mann-Whitney U testleri ANOVA veya Kruskal-Wallis testi ve Ki-kare testi kullanılmıştır. Yüksek SS`un bağımsız değişkenlerinin tespiti amacıyla çoklu lojistik regresyon analizi kullanılmıştır. Bulgular: SS tertilleri arasında yapılan analizde, non-O kan grubunda yüksek SS skoru anlamlı olarak daha fazla izlendi (%56,2'e karşı 75,9'a karşı %80,2, p<0,05). Bununla birlikte, Rh tiplerinin sıklığı tüm gruplarda benzerdi. Yüksek SS prediktörlerini belirlemek için lojistik regresyon analizi uygulandı. Buna göre non-O kan grubu (OR: 2,68, %95 CI 1,65-4,35, p<0,001), sol ventrikül ejeksiyon fraksiyonu (LV-EF) (OR: 0,93, %95 CI 0,91-0,95, p<0,001), düşük yoğunluklu lipoprotein (LDL) (OR: 0,98, %95 CI 0,97-0,99, p<0,001) ve tahmini glomerüler filtrasyon hızı (e-GFR) (OR: 0,99, %95 CI 0,98-0,98, p<0,001) yüksek SS'nin bağımsız prediktörleri olarak bulundu. Sonuç: Bu çalışmada ABO kan grupları ile anjiyografik koroner arter hastalığı kompleksitesi arasında anlamlı ilişkiyi gösterdik.
Objective: We aimed to investigate the relationship between ABO blood groups and complexity of coronary lesions assessed by SYNTAX score (SS) in stable coronary artery disease (CAD) patients. Methods: Our cross-sectional and observational study population consisted of 559 stable CAD patients. From all patients, ABO blood group was determined and the SS was calculated as low SYNTAX score (0-22), intermediate SYNTAX (23-32) score and high SYNTAX score (>32). Statistical analysis was performed using Student’s t-test or Mann-Whitney U test, ANOVA, or Kruskal-Wallis test and chi-square test. Multiple logistic regression analysis was used to identify the independent predictors of high SS. Results: The analysis between the SS tertiles revealed that the frequency of non-O blood group was significantly higher in the upper SS tertiles (56.2% vs. 75.9 vs. 80.2%, p<0.05). However, the frequencies of Rh types were similar in all tertiles. Multiple logistic regression analysis was applied for determining the predictors of high SS. Accordingly, non-O blood group (OR: 2.68, 95% CI 1.65-4.35, p<0.001), LV-EF (OR: 0.93, 95% CI 0.91-0.95, p<0.001), LDL(OR: 0.98, 95% CI 0.97-0.99, p<0.001), and e-GFR (OR: 0.99, 95% CI 0.98-0.98, p<0.001) were found to be the independent predictors of high SS. Conclusion: We showed that there were significant associations between ABO blood groups and complexity of angiographic CAD.

11.The relation between location of paravalvular leakage and time to reoperation after mitral valve replacement

Amaç: Mitral kapak ameliyatı sonrası paravalvüler kaçak ameliyatında kaçağın lokalizasyonunun tanı ile reoperasyon zamanı arasındaki ilişkisi araştırıldı. Yöntemler: Gözlemsel retrospektif bir çalışma planı ile reoperasyona alınan 59 hastadan, beş yıllık klinik ve ekokardiyografik takibi olan 47 hasta çalışmaya dahil edildi. Ekokardiyografik inceleme ile kaçak bölgesinin yerine göre hastalar Grup 1 (Leaflet grubu) ve Grup 2 (Kommisüral) olmak üzere iki gruba ayrıldı. Demografik veriler, preoperatif veriler, re-operasyon nedenleri ve hastanın ilk paravalvüler kaçak tanısı alması ile reoperasyonu arasındaki zaman periyodu kaydedildi. Hasta grupları eşleştirilmiş ve Mann-Whitney U testleri ile değerlendirildi. Bulgular: İlk mitral kapak ameliyatından medyan 180 gün (1 gün-28 yıl) sonra paravalvüler kaçak tanısı kondu. Medyan takip zamanı 5 yıl (1-16 yıl) idi. Yaş, cinsiyet, sol ventrikül ejeksiyon fraksiyonu, kaçak sayısı ve miktarı gruplar arasında farklı değildi (p>0,05). Tanı ile reoperasyon zamanı arasındaki zaman Grup 1’de Grup 2’ye göre daha uzundu (39,0±9,9 vs. 19,5±12,8 ay, p=0,002). Otuz günlük mortalite %4,3 (2/47) idi. Grup 1’de mortalite 2 hastada gözlenirken (2/21, %9,8) Grup 2’de mortalite yoktu (0/26, %0) (p=0,002). Sonuç: Paravalvüler kaçak tanısı konması ile reoperasyona alınma arasında geçen süre, leaflet grubunda kommissür grubuna göre daha uzundu. Biz mitral kapak replasmanı sonrası paravalvüler kaçak gözlenen hastaların takibindeki ekokardiyografik değerlendirmede paravalvüler kaçağın yerinin belirtilmesini öneriyoruz.
Objective: A relation between the location of the paravalvular leakage (PVL) and time to reoperation after mitral mechanical valve replacement was investigated. Methods: In an observational retrospective study plan, from 59 patients who underwent reoperation only 47 patients having clinical and echocardiographic follow-up for five years were included into study. Depending on echocardiographic evaluation of location of leak, patients were divided into Group 1 (Leaflet) and Group 2 (Commissural). Demographics, preoperative variables, causes of reoperation, the time period between diagnosis of PVL and reoperation were recorded. Unpaired t test or Mann-Whitney U test were used for comparison of variables between groups. Results: A PVL was diagnosed after a median time of 180 days (range: 1 day-28 years) after the first mitral valve replacement. The median follow-up period was 5 years (range; 1-16 years). Age, gender, left ventricular ejection function, number and size of leaks did not differ between groups (p>0.05). The time period between diagnosis and reoperation time was longer in Group 1 in comparison to Group 2 (39.0±9.9 vs. 19.5±12.8 months, p=0.002). The 30-day mortality for valve reoperation was 4.3% (2/47). In Group 1, 2 patients (2/21, 9.8%) died whereas, no death was observed in Group 2 (0/26, 0%) (p=0.002). Conclusion: The time period between diagnosis and reoperation was longer in leaflet leak group in comparison to commissural leak group. We suggest echocardiographic evaluation should include location of the paravalvular leakage during follow-up of patients with PVL after mitral valve replacement.

12.Predictive value of aortic knob width for postoperative atrial fibrillation in coronary artery bypass surgery

Amaç: Bu çalışmanın amacı göğüs grafisinde aort topuz genişliğinin (ATG) izole koroner baypass operasyonlarından sonra atriyal fibrilasyon (AF) gelişimi ile ilişkili olup olmadığını araştırmaktır. Yöntemler: Bu retrospektif gözlemsel Kohort çalışmada, hemodinamik olarak önemli kapak problemi olmayan 135 hastayı değerlendirdik. Bilgisayar sistemi yardımıyla ATG göğüs grafisinde ölçüldü. ATG ve postoperatif atriyal fibrilasyon (POAF) gelişimi arasında bağımsız ilişki olup olmadığını bulmak için çoklu regresyon analizi yapıldı. ATG’nin tanı değeri ROC analizi kullanılarak değerlendirildi. Bulgular: POAF tüm hastaların 43’ünde (%31,8) ortaya çıktı. Yaş, ATG, sol atriyum çapı (SA) ve C-reaktif protein (CRP) POAF gelişen grupta, POAF gelişmeyen gruba göre önemli bir şekilde daha yüksekti (67,2±8.6 vs 61.3±9,8 p=0.004; 45,6±5.8 mm vs 36,1±3.8 mm, p<0,001; 37.9±3,5 vs 35,8±3.1 mm, p=0.002 ve 10,6±8,5 vs 5,6±6,5 mg/L p=0.001 sırasıyla). Çoklu lojistik regresyon analizinde ATG, SA çapı ve CRP’ nin POAF gelişiminin bağımsız prediktörleri olarak tespit edildi [OO=4,527, %95 GA=1,315 -15,588, p=0,017; OO=2,834, %95 GA=1,091-7,360, p=0,032 ve OO=1,300, %95 GA=1,038 -1,628, p=0,022 sırasıyla]. ROC analizinde ATG’nin 36,5 mm olması POAF gelişmesini %84,4 duyarlılık ve % 64,6 özgüllük ile tespit edebiliyordu (AUC=0,84, 95% GA=0,75-0,94, p<0,001). Sonuç: Biz izole koroner baypass operasyonlarından sonra gelişen POAF ve ATG arasında önemli bir ilişki olduğunu gösterdik. PA akciğer grafisi ucuz, her zaman ulaşılabilen ve her kalp ve damar cerrahı tarafından kolaylıkla değerlendirilebilen bir klinik araçtır.
Objective: The aim of our study was determine whether aortic knob width (AKW) is associated with the development of atrial fibrillation (AF) after isolated coronary artery bypass surgery (CABG). Methods: In this retrospective observational cohort study, we evaluated 135 patients without hemodynamically significant valvular problems. AKW was measured on chest X-ray by digital system. Multiple logistic regression analysis was used to find independent associates of postoperative AF (POAF). The diagnostic value of AKW was assessed using ROC analysis. Results: POAF occurred in 43 (31.8%) of all patients. The age, AKW, left atrial (LA) diameter and C-reactive protein (CRP) were significantly higher in patients with POAF than without POAF (67.2±8.6 vs 61.3±9.8 years, p=0.004; 45.6±5.8 vs 36.1±3.8 mm, p<0.001; 37.9±3.5 vs 35.8±3.1mm, p=0.002 and 10.6±8.5 vs 5.6±6.5 mg/L, p=0.001 respectively). Multiple logistic regression analysis demonstrated that AKW, LA diameter and CRP were independently associated with POAF (OR=4.527, 95% CI=1.315 -15.588, p=0.017; OR=2.834, 95% CI=1.091-7.360, p=0.032 and OR=1.300, 95% CI=1.038-1.628, p=0.022 respectively). ROC analysis has demonstrated that aortic knob of 36.5 mm constitutes the cut-off value for the occurrence of POAF with 84.4% sensitivity and 64.6% specificity (AUC=0.84, 95% CI=0.75-0.94, p<0.001). Conclusion: We have demonstrated a significant association between the AKW and AF development after isolated CABG. PA chest radiography is a cheap and readily available clinical tool and it can be examined easily by every cardiovascular surgeons.

Akut stres kardiyomiyopati, koroner arter hastalığı olmayan miyokardiyal dokuda aşırı fonksiyonu artmış ve fonksiyonu azalmış bölgeleri içine alan nadir bir hastalıktır. Etkileyici klinik seyrindeki kendine özgü bölgesel özelliği akut gelişen uyarıcı stres altında sol ventrikül (SV) bazalinin aşırı artmış fonksiyonunu içerir. Stres uyarısının son bulmasından sonra fonksiyon bozukluğuna sahip olan midapikal bölge tam bir fonksiyonel doku iyileşme süreci geçirir. Akut stres kardiyomiyopatide SV yeniden şekillenmesinin geriye dönüş evrimi, 2 ve 3 boyutlu ekokardiyografi kullanılarak literatürde tanımlanmıştır. Akut stres kardiyomiyopatide SV yeniden şekillenmesiyle ilgili bu ikinci raporda daha çok fonksiyon bozukluğu olan SV miyokardiyal dokusunun tam geriye dönüşüne yol açan altta yatan mekanizmaları inceledik. Böylece bu akut klinik antitede miyokardiyal doku iyileşmesinin öngörüsü için faydalı olabilecek stres altında bölgesel miyokardiyumun korunmuş veya aşırı fonksiyon varlığına odaklandık. Yeniden şekillenmenin tam geriye dönüşümüne potansiyel katkı verebilecek kısa süreli hastalık seyri ve kalıcı doku hasarına yol açan kronik hastalıkların ağır tutulumundan farklı olarak önceki hastalık ataklarının olmayışını da tartıştık.
Acute stress cardiomyopathy is the unique disease which represents exaggerated and dysfunctional regions of the same cardiac tissue at the same episode. The impressive clinical course which involves the specific region is the stress-mediated exaggerated function of LV base under acutely developed stress induction. After abolishment of stress induction, dysfunctional part of LV which is the midapical myocardium undergoes a complete tissue functional recovery. The evolution of reverse remodeling in acute stress cardiomyopathy has been described using 2 and 3-dimensional echocardiography in the literature. This is the second report regarding reverse LV remodeling in acute stress cardiomyopathy in which we rather evaluate the underlying mechanisms leading complete reverse LV remodeling of dysfunctional myocardium. Therefore, we focus on the existence of preserved and exaggerated regional tissue under stress which possibly represents the predicted myocardial tissue recovery in this acute clinical entity. We also discuss the potential contribution of short-term disease course and lack of prior disease episodes to complete reverse remodeling differently from the heavy burden of chronic diseases leading to permanent tissue jeopardy.

Kalıcı kalp pili ve takılabilir kardioverter defibrilatör teknolojisindeki gelişmeler sayesinde kardiyak hastalıkların tedavisinde yerleştirilebilir cihaz kullanımı artmaktadır. Cihaz ile ilişkili enfeksiyonlar da buna paralel olarak artış göstermekte ve önemli bir klinik problem olmaya devam etmektedir. Cihaz ile ilişkili enfeksiyonların doğru tanı ve optimal tedavi alması kolay olmamaktadır. Tedavi, cihazın tamamen çıkarılması ile uzamış antibiyoterapi sürecini kapsamaktadır ve mikrobiyoloji ile görüntüleme tekniklerinin desteğini içeren multidisipliner bir yaklaşım gerektirmektedir. Bu yazı, cihaz ile ilişkili enfeksiyon patogenezinin, altta yatan risk faktörlerinin, tanı ve tedavide öne çıkan güncel gelişmelerin altını çizmektedir.
The use of implantable devices in management of cardiac diseases is increasing as a result of improvements in technology of permanent pacemaker and implantable cardioverter defibrillators. Device related infections are also rising accordingly and have become an important clinical problem. Accurate diagnosis and optimal management of these infections is challenging, necessitating complete removal of the device and prolonged antibiotic therapy. In this regard, a multidisciplinary approach is required with the essential support of microbiology and imaging modalities.
